At the consultation, Dr Goldman will discuss your desired results and give you a clear understanding of what to expect. While Dr Goldman is happy to show you before and after photos of women who have had breast augmentations, it’s important to remember that all bodies are different and what works for some women may not be realistic for others. Dr Goldman will discuss this at the consultation.
Choosing the right size and shape of breast implants is a partnership between you and the cosmetic surgeon; there are a number of important steps that you will go through during the consultation to determine the best breast size for you. These steps include detailed measurements of your chest and breasts, reviewing a series of before and after photographs of patients, and viewing the complete range of breast implants in their various shapes and sizes.
The procedure usually takes between one and two hours. An incision is first made, a pocket is then carefully created and the breast implant is inserted. The pocket is then carefully modified to accommodate the breast implant. The incision is then closed with sutures.
The positioning of implants over or under the pectoral muscles is controversial amongst Plastic and Cosmetic Surgeons. Dr Goldman now almost exclusively places implants over the muscle, not beneath. The subglandular placement position lifts low-lying nipples more effectively than the submuscular position. Dr Goldman will explain these positioning aspects in detail during your consultation.

In the vast majority of cases, Dr Goldman prefers the Inframammary method as this method has a number of advantages to the patient. Regardless of the approach chosen, the incisions are relatively small and generally heal extremely well.

Firstly, you will be fitted with a specialised support bra that you will need to purchase from a preferred supplier. It will support the breasts in a comfortable healing position. We request that you wear this bra at all times, except for showering, for approximately 4 weeks. We will provide you with medication for any discomfort. Most patients do not need anything for pain 6 to 7 days after surgery.
During the first week after breast augmentation surgery, you will need to take life very easy, and avoid heavy lifting and excessive movements of your arms. Activities can be gradually increased after the first week, within your own comfort level. You will be able to shower 5 days after your surgery. You should be able to return to your work 1 week after your surgery. You will need to avoid aerobics and other strenuous exercise for about six weeks after your surgery. It is recommended that air travel be avoided for the first two weeks after surgery. However, if you have any specific concerns, you should discuss them with your Cosmetic Surgeon, Dr Goldman.
For the first few weeks after breast augmentation surgery, it is possible that you may feel that your breast implants are “too big” or perhaps too tight. This will be due to the swelling from the breast enlargement surgery. After a few weeks, this swelling settles. You will also have become more accustomed to your new breast size at that time. Some women report that their nipples become over-sensitive or under-sensitive following breast augmentation surgery. These findings are most common during the first few weeks after breast implant surgery. They are usually temporary.
There will be a review consultation organised for you to see Dr Goldman 14 days after your breast augmentation surgery. Further appointments will be set up for you to see Dr Goldman both 6 weeks and 3 months from the date of your cosmetic surgery.
Breast Augmentation is a relatively straightforward procedure but as with any operation, there are risks associated with surgery and complications specific to this procedure.
A well known and publicised problem is capsular contracture. When a breast implant is inserted, a scar capsule forms around it as part of the natural healing process. In 2% to 5% of cases, the capsule may sometimes tighten and compress the breast implant, causing the breast to feel firmer than normal. Capsular contracture can occur to varying degrees. If it is severe, it can cause discomfort or changes in the breast’s appearance. In such cases, more surgery may be needed to modify or remove the scar tissue, or perhaps remove or replace the breast implant. Dr Goldman has a number of methods to minimize capsular contracture and he will explain this to you during your consultation.
As with any surgical procedure, although uncommon, excessive bleeding following breast augmentation surgery may cause swelling and pain. A small percentage of women may develop an infection around the breast implant. Whilst the majority of women do not experience complications, you should discuss with Dr Goldman any concerns that you may have in this regard.
