Each procedure is unique to the individual and will demand a holistic approach. In general, here is an outline of what to expect from start to finish.
At the consultation, Dr Goldman will discuss your desired results and give you a clear understanding of what to expect. While Dr Goldman is happy to show you before and after photos of women who have had breast augmentations, it’s important to remember that all bodies are different and what works for some women may not be realistic for others. Dr Goldman will discuss this at the consultation.
Choosing the right size and shape of breast implants is a partnership between you and your specialist cosmetic surgeon. There are a number of important steps that you will go through during the consultation to determine the best breast size for you. These steps include detailed measurements of your chest and breasts, the option of reviewing before and after photographs of previous patients and viewing the complete range of breast implants in their various shapes and sizes.
The procedure usually takes between one and two hours. An incision is first made, a pocket is then created and the breast implant is inserted. The pocket is carefully modified to accommodate the breast implant. The incision is then closed with sutures. Remember, all sutures are internal and dissolving.
The positioning of implants over or under the pectoral muscles is controversial amongst Plastic and Specialist Surgeons. Dr Goldman now almost exclusively places implants over the muscle, not beneath. The subglandular placement position lifts low-lying nipples more effectively than the submuscular position. Dr Goldman will explain these positioning aspects in detail during your consultation.
There are three possible incisions methods that can be used in Breast Augmentation (Breast Implants) surgery:
- Transaxillary: under the arm
- Inframammary: under the breast just below the breast fold
- Periareolar: around the lower edge of the areola
In most cases, Dr Goldman prefers the Inframammary method as this method has several advantages to the patient. Regardless of the approach chosen, the incisions are relatively small and generally heal extremely well.

Firstly, you will be fitted with a specialised support bra that you will need to purchase from a preferred supplier. It will support the breasts in a comfortable healing position. We request that you wear this bra at all times, except for showering, for approximately 4 weeks. We will provide you with medication for any discomfort. Most patients do not need anything for pain 6 to 7 days after surgery.
During the first week after breast enlargement surgery, you will need to take life very easy, and avoid heavy lifting and excessive movements of your arms. Activities can be gradually increased after the first week, within your own comfort level. You will be able to shower 5 days after your surgery. You should be able to return to your work 1 week after your surgery. You will need to avoid aerobics and other strenuous exercise for about six weeks after your surgery. It is recommended that air travel be avoided for the first two weeks after surgery. However, if you have any specific concerns, you should discuss them with your Specialist Surgeon, Dr Goldman.
For the first few weeks after breast augmentation surgery, it is possible that you may feel that your breast implants are “too big” or perhaps too tight. This will be due to the swelling from the breast enlargement surgery. After a few weeks, this swelling settles. You will also have become more accustomed to your new breast size at that time. Some women report that their nipples become over-sensitive or under-sensitive following breast augmentation surgery. These findings are most common during the first few weeks after breast implant surgery. They are usually temporary.
A review consultation is organised for you to see Dr Goldman 14 days after your breast augmentation surgery. To ensure your long-term health remains at its best, an after-care program is organised with meetings at 6 weeks and 3 months from the date of your surgery. These appointments will also include breast cancer checks. This is all part of the holistic approach that is offered by Dr Goldman.
Breast Augmentation is a relatively straightforward procedure but as with any operation, there are risks associated with surgery and complications specific to this procedure.
A well known and publicised problem is capsular contracture. When a breast implant is inserted, a scar capsule forms around it as part of the natural healing process. In 2% to 5% of cases, the capsule may sometimes tighten and compress the breast implant, causing the breast to feel firmer than normal. Capsular contracture can occur to varying degrees. If it is severe, it can cause discomfort or changes in the breast’s appearance. In such cases, more surgery may be needed to modify or remove the scar tissue, or perhaps remove or replace the breast implant. Dr Goldman has a number of methods to minimize capsular contracture and he will explain this to you during your consultation.
